Nursing Process from the self-care of an Elderly in the clinical setting

Introduction: This condition involves the dilation of veins around the esophagus due to an excess of blood flow from the stomach. When the blood cannot reach the heart, it seeks alternative routes, causing these veins to dilate, potentially rupture, and bleed. Methodology: This case study utilized the Nursing Care Process to address an elderly patient with esophageal varices. For assessment, a tool based on Orem's General Theory of Self-Care was used. Diagnostic deduction was performed using NANDA I taxonomy, outcomes were selected from the NOC taxonomy, and interventions were based on Clinical Practice Guidelines in Nursing. Results: The study focused on a 68-year-old patient with type 2 diabetes, hypertension, and esophageal varices. Diagnostic deduction identified five nursing diagnoses: two actual and three at-risk. The affected universal self-care requisites were universal self-care requisites and developmental self-care requisites. Six care plans were developed, incorporating evidence-based nursing interventions from three Clinical Practice Guidelines. Conclusions: Evidence-based nursing interventions with a focus on health education provide comprehensive care and help patients with type 2 diabetes improve disease control.
